How to order:  Either email or fax the hot sheet(s) to the email or fax# on the form.  It's that simple!  Please be sure to fax the new customer form the first time you order.

Please include your name, store name and complete address (or website if selling on-line), phone #, fax #, email address and your tax resale # (you don't need one if you are in a state w/o sales tax such as Oregon or Delaware).
